Deadline to submit MANDATORY FORM Online has passed.  

 

Deadline to submit MANDATORY FORM by Mail via the PO Box has passed.​

MANDATORY FORM can be printed and HAND DELIVERED TO PHS OFFICE until Thursday June 21 at NOON

OR - Your Graduate must bring the completed/signed hard copy they received during graduation rehearsal on their chair to GRADNITE 2018.   If your graduate has lost their copy - Copies will be available at GradNite Drop-off- but a parent/guardian must accompany the graduate inside to sign the forms.

 

Please DO NOT go through the regular check-in line - ask to see the Medical personal  in the athletic directors office - first to submit your forms, then proceed through regular check- in.
